Medical transmission record by KST

The Medical Transmission Form, developed by Khimaira Strategy Tactics.

With the aim of ensuring the proper transmission of information to the final care structure, this form will be relevant in any IFAK or first aid kit.
This guide was designed for the simplest and most intuitive use possible, including by non-medical personnel who may find themselves in a first-response role. Its underlying principle was to optimize the operator's performance by avoiding overwhelming them with unnecessary information, through a clear presentation and as many cognitive aids as possible.
Adapted to single-victim situations, as well as Multiple Victims (NOVI), it includes decision-making aids for triage upon arrival at the scene, and for categorizing the level of urgency for evacuation.
The completion of this form relies as much as possible on placing crosses in boxes, to best guide the operator under intense stress without requiring them to use their memory with free filling.
A silhouette in a reference anatomical position allows for the rapid description of the lesions encountered.
On the back, the complete report is organized in a table constituting an "ATMIST" evacuation request for:
- Victim's age
- Time: time of injury
- Injury mechanism: fall, road traffic accident, gunshot wound, etc…
- Injury: type of injury (left leg, chest, etc.)
- Symptoms
- Treatments
This standardized health messaging system is therefore ready to be read by the operator who reports to their superiors, at the 15 or 18 call center, for a clear and effective call.
The sub-sections "symptoms" and "treatments" are themselves organized within a table that includes all the authorized/necessary procedures, following the order of the MARKET, an acronym for the management of severely traumatized injured people.

Perforated at the level of the circle at the top of the card, it can be slipped onto the wrist or ankle of any victim receiving care using a simple elastic band. This will allow for the efficient transmission of information on the injuries observed and the actions taken to the final healthcare facility, facilitating communication between the fire/emergency medical teams (SMUR) likely to provide transport.

Rigid and plastic-coated, it will be durable over time and will withstand being carried folded in an IFAK despite the movements and handling of the operator.
